I'm sorry to hear about Gravy. Feline Lower Urinary Tract disease (a term incorporating various bladder related conditions in cats - cystitis, bladder stones, blocked bladders...) can be a very frustrating condition to control, and it does sound like Gravy has had extensive investigations. A penile urethrotomy is an operation performed when a cat continually gets blocked bladders. The aim is to remove the narrowest portion of the urethra to prevent blockages occurring. Sometimes this works well, but it doesn't work in 100% of cases.
If he still has cystitis, then I would want to check whether this is 'idiopathic' meaning there is no obvious cause for it, or whether there is a reason such as bladder stones. There are prescription urinary diets available to help prevent the formation of urine crystals (which form stones) - this may be worth looking in to if this is the cause of the cystitis.
In terms of medications, I would always try:
1) Antiinflammatories such as Meloxicam (Metacam/Loxicom) - an oral solution given daily
2) Bladder/urethra muscle relaxants such as Dantrium and hypovase (daily tablet/solution)
3) Cystaid - sustains the production of glycosaminoglycans to help maintain the protective bladder lining.
Feliway can certainly help. Also try a cat water fountain to encourage drinking, and try placing litter trays in every room in the house.

Thank you for your response, Gravy has Struvite crystals and is on special food, he refuses to eat the wet food so he is on the royal canin urinary s/o i think it is, hes got metacam but its a battle to get him to take it, he sometimes goes a week without weeing, then he will wee every day or every other day as it is this week. One reason the vet thinks he wont go to his tray is that he associates it with pain, he is constantly biting where he was operated on (self mutilation) i think my vet called it and obviously when he does wee its very painful, he runs around the house going mad when he wees, its like he has no control as he has weed on me, on my sofa, my bed, living room table.
I will ask my vet about the bladdder relaxents although im pretty sure hes had them, im lucky i have a very good vet who has put a lot of time and effort into helping gravy and hes always saying let try this 1 last thing as he knows i dont want to loose him

Hi Wendy
Your cat may now have associated the litter tray with feeling discomfort. I would try experimenting with some alternative types of trays ie low sided (more like a seed tray) or uncovered if you normally have a covered one. Also offer a different choice of substrate and depth of substrate.
